- A referral from your doctor is required to be eligible for a Medicare rebate. Please check you have an up-to-date referral.
- Please bring a current medication list and any relevant results to your appointment.
- Please bring your Medicare card, Concession card and/or DVA card.
- Paid parking is available next to the Midland Specialist Centre.
- If you need to cancel your appointment please phone us at your earliest convenience. Please note a cancellation fee may be charged if you cancel your appointment without sufficient notice.
